TrueForm is a medical aesthetics and wellness clinic offering cosmetic treatments, skincare services, and related products.

2. Client Eligibility

Our treatments are available to clients who are at least 18 years of age.

Before receiving any service, you are responsible for providing complete and accurate information about your health, medications, allergies, and medical history. Some treatments may not be appropriate depending on your individual circumstances, and we reserve the right to decline treatment when necessary.

3. Services

Every treatment begins with your individual needs in mind. Recommendations and treatment plans are tailored to each client, and no two experiences are exactly alike.

Because every individual responds differently, treatment outcomes cannot be guaranteed. TrueForm reserves the right to refuse, postpone, or discontinue any service if it is considered medically inappropriate or if proceeding would compromise client safety.

4. Health & Treatment Disclaimer

Our services are intended to enhance appearance and support skin health, but they do not replace professional medical advice or healthcare.

By choosing to proceed with treatment, you understand and accept that:

• Cosmetic procedures involve potential risks and possible side effects.

• Results differ from one individual to another.

• Honest and complete disclosure of your medical history is essential for safe treatment.

Certain procedures may require a consultation or medical clearance before treatment can proceed.

5. Booking Appointments

Appointments can be scheduled through our online booking system or by any other booking method offered by TrueForm.

When making a reservation, you agree to:

• Select the service that best matches your needs.

• Provide accurate personal and contact information.

• Follow all preparation and aftercare instructions provided by our team.

Your appointment time is reserved exclusively for you.

6. Pricing & Payment

Payment may be collected either when your appointment is booked or at the time services are provided.

Treatment fees, retail pricing, and promotional offers may change at any time without prior notice.

Accepted payment methods will be communicated during the booking process.

7. Appointment Changes, Deposits & Refunds

Some appointments require a booking deposit or a valid credit card to secure your reservation.

Unless otherwise specified, deposits are non-refundable but will be credited toward your scheduled treatment.

If you need to reschedule or cancel your appointment, we ask that you provide at least 48 hours' notice. Appointments changed with less than 48 hours' notice may be subject to a cancellation fee or loss of the booking deposit. Missed appointments without notice may incur a higher fee.

Due to the nature of aesthetic services, completed treatments cannot be refunded.

Retail products are considered final sale once opened. Unopened products, in their original condition, may qualify for an exchange or store credit at our discretion.

8. Packages, Memberships & Special Offers

Treatment packages, memberships, gift certificates, and promotional pricing are subject to the specific terms communicated at the time of purchase.

Unless otherwise stated:

• Offers cannot be combined.

• Packages and memberships cannot be transferred to another individual.

• Promotional pricing may be modified or withdrawn at any time.

• Expiry dates may apply to prepaid services.

9. Treatment Expectations

Every client's skin, health history, and lifestyle are unique.

For this reason:

• Individual outcomes will vary.

• Multiple treatments may be necessary to achieve optimal results.

• Maintenance sessions may be recommended and are not included unless specifically stated.

Receiving treatment does not guarantee a particular cosmetic result.

10. Ownership of Content

Everything displayed on the TrueForm website—including written content, branding, logos, graphics, photographs, videos, and design elements—belongs to TrueForm unless otherwise noted.

No material may be copied, reproduced, distributed, or used without prior written permission.

11. Website Conduct

By using this website, you agree to use it responsibly and lawfully.

You may not:

• Attempt to gain unauthorized access to our systems.

• Submit inaccurate or fraudulent information.

• Disrupt the operation or security of the website.

These Terms & Conditions are governed by the laws of the Province of Ontario and the applicable laws of Canada.

TrueForm may revise these Terms & Conditions from time to time to reflect changes in our services, policies, or legal obligations.

The most current version will always be posted on our website, and continued use of our services constitutes acceptance of any updates.
